Dentist Brain Candy

The podcast is designed for the dentist who likes to learn and improve their practice. The information and content delivered is designed to help the restorative dentist move their practice forward. Each week we start off with an quote that gets the metal gears turning, a book review which will be digested for the listener over a series of episodes pulling out relevant material and applying it to the dental practice. Next an article will be reviewed from the literature and available for download. The show will close with an interesting case that I have come across. Pictures and radiographs, when appropriate will be on the website to review as well. The benefits include having a virtual dental buddy that you can listen to and “hang out with” and talk shop and grow your experience, knowledge and enthusiasm for our profession.

In this episode, I discuss the misdiagnosis of a swelling on the roof of the mouth that turned out to be cancer. This was misdiagnosed twice and can have serious implications for patients. In addition, we talked about the incidental radiographic finding with a panorex showing calcifications in the Carotid artery and its association with obstructive sleep apnea heart attacks and strokes. We also discuss using dental implants to help a patient with severe erosive lichen planus. Lichen planus can be a painful mucosal condition making Dentures very difficult or impossible to wear. Dental implants offers the opportunity to elevate the appliance off of the mucosa making impossible to wear Prosthetics without pain.

This week I discuss articles out of the Journal of Oral & Maxillofacial Surgery in December of 2018. We will discuss a pork larvae infestation of the oral mucosa in three separate patients including both the clinical presentation and management of these lesions.  We shall discuss how to prevent periodontal defects distal to mandibular third molars after the extraction of wisdom teeth as well as how to predict the recurrence of regional disease after primary resection of oral squamous cell carcinoma. In addition to this, we shall also discuss the utility of viewing a third molar video particularly a third molar informed consent video and whether this helps to decrease patients anxiety and whether an Emergency Room Physicians are able to accurately diagnose, order CT scans and consult for specialists for head and neck infections and particularly odontogenic infections.

Botox originally was used for noncosmetic purposes for 13 years and then transitioned into cosmetic uses for lines and wrinkles of the face particularly in the glabella frontalis and crow's-feet areas on the face to eliminate wrinkling and specifically Dynamic wrinkles. There are many other uses within dentistry in every dentist should be aware of these uses and ideally Implement an offer these services to your patients. The administration of Botox is well within the scope of practice and I think dentist, in general, are fantastic with giving shots and should consider offering the service with the proper training. In this episode, I will discuss other uses of Botox in dentistry and I think you'll find this fascinating and enlightening

In this episode, I discuss the full-day presentation and the high points from Fred Joyal's book and presentation on becoming remarkable.  He did a fantastic job and there's some great high points and I also go through and just give a breakdown of the different chapters that he has in his book. Everyone that came to the presentation received the free book as well. Our next continuing education event is February 8th 2019 when we will have OSHA HIPAA CPR renewal as well as a one-hour talk on Botox and fillers as well as the opportunity to receive Botox and fillers which will be a blast. There is now an alternative fixed restoration that is not a hybrid restoration. Zest International has developed an abutment and a system called the locator FTX system. In this episode we discuss using four or more implants in either the maxilla or mandible and specifically 13 cases that were studied for 3 years showing that this is a successful and reasonable treatment modality. Anybody treating patients with fixed full Arch Restorations needs to be aware of this treatment concept.

We've all had or soon to have a patient with a fixed restoration on four more implants and one of those key implant fails. In the past the only choice and option available was to toss out the final restoration and place a new implant and make a brand new fixed hybrid restoration.  In this episode, I discussed a novel new treatment approach that is a salvage procedure so that the final restoration does not need to be remade.

And then discuss whether or not we need to be concerned about our cancer patients that have central venous catheters or Central lines and whether or not a dental cleaning can lead to infection of these catheters within go into a discussion on oral mandibular dystonia both the recognition and treatment of that and we discuss a severe complication of prescribing a tetracycline antibiotic to a 14-year-old boy.

This week I start the discussion off talking about solar navigation and three different ways that you can determine Direction based on the Sun.  This expands app on last week's discussion of celestial navigation I didn't go into whether there's a link between kidney stones and salivary Stones we discussed virtual reality in surgical training and a group out of England that has created a virtual surgical training program we also discuss the use of intraoperative computed tomography or basically a CT scan in facial trauma surgery and whether or not an abnormal swallow pattern sometimes called an infantile swallow can lead to an increase probability of relapse in orthognathic surgery and with relapse if there's more likely to be an abnormal swallow pattern this week is jam-packed full of information so make sure you tune in and enjoy.

In this episode I break down to interesting topics. The first is discussing triggers with oral lichen planus. Patience with biopsy proven oral lichen planus we're given a questionnaire in this Prospect of study to see what the triggers were and if avoidance of the triggers was helpful for these patients. A list of the typical and most common triggers is also given for your review. I also discuss the odds and likelihood and reasons for plate removal with jaw surgery. This discussion will help you have a better feel for when patients will have plates removed and if they are likely to have an improvement in signs and symptoms with removal of those plates.

In this episode I discuss what the prognostic factors are for persistent diplopia after orbital blowout fractures. I also discuss an article that did a benchtop study on the neurotoxicity of articaine versus lidocaine and found that there was no difference in toxicity which would suggest that Articaine may be acceptable for inferior alveolar or mandibular nerve blocks

Their survey asks questions about prescribing patterns and attitudes of oral surgeons. Over 300 people responded to the survey and the results are quite interesting.
