This week I go over two articles that are very relevant to everyone in dentistry today. Lichen planus, especially the erosive variety is very annoying and affects the quality of life for patients and we can help them. Anyone doing 3rd molar surgery or seeing patients and referring them for wisdom teeth should and now will know that flap design does not affect the health of the second molar long term.
I start off with an Irish Proverb for the quote of the day just to get everyone loosened up and then discuss the fifth and final installment of Wheelbarrow Profits by Jake and Gino. Right now the market is hot for Multi-unit Family Investing and so getting in may not be a good time but learning about this is never a bad time. The article about lip cancer and specifically about squamous cell carcinoma is over a 25-year span and has some solid take home points. The interesting case is that of a traumatically fracture tooth number 9 and a dentoalveolar fracture as well and how I approached the treatment with a significant gingival injury.

In this episode, I continue with our book review of Wheelbarrow Profits and then move to a very interesting study that used 5% Fluorouracil instead Modified Carnoy’s solution to decrease the recurrence rate of keratocystic odontogenic tumors with a decrease in paresthesias. In my experience, only two implant systems excel at preserving crestal bone. Astra and Ankylos implants are the only two that I have seen published and personal results with preserved marginal bone. This week I delve into several excellent articles and break down the data on marginal bone loss. I currently place mostly Nobel implants followed by Astra implants. One article shows that after the one year mark the bone loss for an Astra implant was the same as a Branemark implant but the key is that this was after close to 1.75mm of bone loss with the Branemark vs the 0.4mm of bone loss with the Astra implant

After injury to the orbital bony vault a common sequelae is enophthalmos. This article reviews a way of assessing patients radiographically to predict when enophthalmos will occur and when to do surgery.
After any surgery including oral surgery postoperative shivering can occur. These patients may be hypothermic but they may not be as well. This article review does a nice job of going into the incidence, causes, risks and solutions for this common occurrence.

I go into a clever tool called an SRT which is used to retrieve a stripped screw head in an abutment. Photos of the tool and a screw I removed are in the interesting cases section as well. The article review discusses custom molar healing abutments in order to obtain ideal soft tissue contours in the final restoration of molar implants.

There are many variations and each has a clinical implication specifically on achieving adequate local anesthetic. I then review a study comparing topical refrigerant vs the traditional 20% topical benzocaine for palatal injections done by a friend of mine Dr Dimarco.
