Dentist Brain Candy

The podcast is designed for the dentist who likes to learn and improve their practice. The information and content delivered is designed to help the restorative dentist move their practice forward. Each week we start off with an quote that gets the metal gears turning, a book review which will be digested for the listener over a series of episodes pulling out relevant material and applying it to the dental practice. Next an article will be reviewed from the literature and available for download. The show will close with an interesting case that I have come across. Pictures and radiographs, when appropriate will be on the website to review as well. The benefits include having a virtual dental buddy that you can listen to and “hang out with” and talk shop and grow your experience, knowledge and enthusiasm for our profession.

This week starts off with two killer quotes by Anthony Robbins.  Next week we will start a new book by Tony so i thought it would be fitting to hear from him today to warm us up to his way of thinking.  The final installment of Crucial Conversations will not disappoint and finish this book.  Telemedicine or remote consults using the internet is used widely in medicine but not much in dentistry and Oral Surgery.  This article goes over the definition of Telemedicine, the benefits and the barriers to entry and costs involved with getting started with Telemedicine.  The interesting case is the All On Four in the maxilla with some great radiographics on the website in the interesting cases section.

This week I chat with Patrick Jenkins, a friend of mine, and a wealth management advisor.  We talk about the importance of being aware of tax implication of investments and how that erode and even destroy returns.  We discuss how with 10% historic returns with the S and P 500 that if taxes were not managed appropriately the real return on your investment is 0% (or worse).  This starting news and advice on what to do about it is revealed today.

Last week was so good with John Maxwell I decided to do more from this fella.  The fourth installment of Crucial Conversations is discussed and then an article on protocols for anticoagulants on dental and oral surgery patients is reviewed.  The interesting case is about a young lady with congenitally missing lateral incisors and the bone grafting technique using Augma to replace this missing bone.

Hybrid restorations are a fantastic restoration that every restorative dentist should be doing. Every dentist (almost) has the skills, talent and knowledge to pull these restorations off but short change their patients by not getting them done.  In this episode, this jagged little pill, will be swallowed gently with an interview with Scott Oyler, a friend of mine, who has been in the dental implant industry for years and has seen and heard it all.  I think you will really enjoy Scott dispelling and discussing these myths on todays episode.

QOD by Charles Swindoll and his quotes highlights that our perceptions dictated more on the path of life than the actual events that happen to us.  The final installment of the book review by Keller and Papasan talks about the four thieves and is mandatory listening for anyone who wants to maximize productivity.  The article I reviewed is about using corticotomies to assist orthodontic tooth movement and is a meta analysis of the literature showing some support and validity to the procedure but that more research is required.  The interesting case is of a fella that was driving his antique car without a seatbelt and got caught in some Pacific Northwest slush and smashed his lower front teeth on the steering wheel of his car.  He lost those teeth and bone in the area and had some pretty nasty lacerations closed as well.

QOD by Jim Rohn is about seizing our day before our day wastes away and then the book review of The One Thing is continued and really dives deeper into happiness and maximizing your time with Block Time.  Block Time must be set and cherished and never compromised to maximize productivity.  For the interesting case I show a patient with impacted canines that was referred for expose and bond where the teeth were exposed and a gold chain and bracket were bonded onto the tooth so the canines can be pulled into the arch.

QOD by Ralph Waldo Emerson is about how the world is full of open gates and opportunities.  Part 2 of The One Thing dives even deeper into the six lies  This book was so rewarding and fun to read and powerful and guaranteed to help you focus on big success.  In the interesting case I show a 3D scan of a future implant site and then the site filled with the implant.  The value of getting and having a scan of an implant site in a lower molar site is highlighted nicely here.

QOD is by Thomas Edison where he talks about how capable we are.  This is the first part of the book review of The One Thing by Gary Keller and Jay Papasan.  Gary Keller is the self made billionaire who made his fortune from Keller-Williams real estate and this book is full of advice on how to achieve big results.  In the interesting case I show pictures of a ridiculously curved root on a maxillary wisdom tooth that I extracted (luckily) in one piece.

QOD by Eva Long, Part 3 of Rework by Fried and Hansson talks about building an audience, hiring at the right speed, and how resumes are faked and how to hire the right person.  The article review is about mouth rinses and choices and how they affect growth factors.  The interesting case is about a typical scenario where a  root canal can go wrong and a perforation happens and then replacing that second molar with and immediate implant.  Since the molar was the correct dimensions I was able to place the implant immediately.

We start off with a double quote for today and then dig into the second part of the book review of Rework by Jason Fried and David Heinemeier Hansson.  There are some really good tips in this book that I know will help your business.  We want to make sure we don’t call out other dental practices.  Do birth control pills increase the incidence of dry socket?  We will review a meta analysis that summarizes the current literature.  The interesting case we have today is an Odontogenic Keratocyst, which has been renamed by the WHO as Odontogenic Keratotumor, in a young lady with basal cell nevus syndrome.

In this episode we begin with a snappy quote from Oscar Wilde which was inspired after watching Point Break in the theatre.  I then review an article that compare the quality of life (QOL) prospectively for two implants or four implants in the mandible.  They made each patient experience two implants and an overdenture and then four implants and an overdenture and vice versa. REWORK by Jason Fried and David Heinemeier Hansson was then started with over ⅓ of the book reviewed and summarized.  The case presentation today was about a really sweet lady that had been in a severe car accident.  She was repaired before originally by another surgeon and came to me for evaluation and reconstruction. I repaired the facial deformities by reconstruction the orbital vault first for the enophthalmos and vertical dystopia.  I then used a CT to mirror image the unaffected side and make a custom facial implant out of PEEK.  The patient also required a BSSO for the mandible deformity.

In this episode medication related osteonecrosis of the jaws is discussed in detail.  Based on the latest position paper by AAOMS, a detailed discussion of the medications that can cause this necrotic bone, the diagnosis and staging and then treatment of the disease is reviewed.  I think anyone doing dentistry today needs to be aware of this material, especially developing a risk profile and treatment protocol in your office.

We start off with two quotes from American proverbs and then continue on the breakdown of Good to Great by Jim Collins digging deeper on the Hedgehog Concept.  Chapter 6 is focused on a culture of discipline on how to create that environment in your practice.  Create a STOP DOING list.  Technology accelerators are also discussed.  How do you evaluate tech?  960 B dollars of debt has gone up greatly since the last survey and this review goes through student debt and the influence this debt has on choices for new dentists.  The case review is on a case of excess gingival tissue around an anterior dental implant and resolution using the CO2 laser.

We start the episode with quote from JRR Tolkien.  Good to Great book review goes into the importance of addressing the brutal facts honestly and having leadership that allows for transparency.  The Hedgehog concept may be the most important concept of the book and we dive deep into this.  Central Giant Cell Granulomas can be treated with Interferon and this article review goes into this modality along with characteristics of CGCG.  The interesting case is a case of CGCG and video of the actual curettage and a discussion ensue.
