This week I start off with no less than three Polish Proverbs! That's right three! We then move on the Part 2 of Meetings Suck by Cameron Herold. From there I discuss Safety Concepts in Anesthetic Management with an emphasis on Ketamine. The interesting case is that of a custom healing abutment that I fabricated with extraction of number 8 and an immediate implant.
In this episode, I start off with a Bulgarian quote of the day because I know one quote last week is just not enough. After a few I cemented in getting things started with a Bulgarian “of the day. I then move on to Part 1 of the book review of Meeting Suck by Cameron Harold. Anybody in a work environment either has to hold meetings Or go to meetings and everybody needs to listen and know this information. I then move on to a literature review from the Journal of Oral and Maxillofacial Surgery on central giant cell granulomas. This gets us all up to date on some of the new novel approaches and factoids in a very direct, rapid fire manner. The interesting case is worth a visit to the website to check out the pictures and radiographs. This crazy wisdom tooth with a large cyst was up at the floor of the orbit on the right side and was removed, biopsied and came back as a dentigerous cyst. Patient is fantastic during and after the procedure.
Thursday of last week was an interesting day and I thought I’d share what I was up to that day. I completed a Le Fort I procedure and did a modification of her previously LeFort I that needed some expansion after some poor healing. The cuts of wisdom teeth as well and then I had an interesting trauma case that ATV accident with a right zygoma and the right mandible fracture.

This week I go over two articles that are very relevant to everyone in dentistry today. Lichen planus, especially the erosive variety is very annoying and affects the quality of life for patients and we can help them. Anyone doing 3rd molar surgery or seeing patients and referring them for wisdom teeth should and now will know that flap design does not affect the health of the second molar long term.
I start off with an Irish Proverb for the quote of the day just to get everyone loosened up and then discuss the fifth and final installment of Wheelbarrow Profits by Jake and Gino. Right now the market is hot for Multi-unit Family Investing and so getting in may not be a good time but learning about this is never a bad time. The article about lip cancer and specifically about squamous cell carcinoma is over a 25-year span and has some solid take home points. The interesting case is that of a traumatically fracture tooth number 9 and a dentoalveolar fracture as well and how I approached the treatment with a significant gingival injury.

This week I have several items to discuss including a new company that is making a 3D identical titanium and Zirconium dental implant that is designed to fill the socket of an immediately extracted tooth. In this episode, I continue with our book review of Wheelbarrow Profits and then move to a very interesting study that used 5% Fluorouracil instead Modified Carnoy’s solution to decrease the recurrence rate of keratocystic odontogenic tumors with a decrease in paresthesias. If you treat these lesions you will not want to miss this discussion. The Interesting Case shows the surgical approach to removal of a mandibular bicuspid that is an extra tooth. In my experience, only two implant systems excel at preserving crestal bone. Astra and Ankylos implants are the only two that I have seen published and personal results with preserved marginal bone. This week I delve into several excellent articles and break down the data on marginal bone loss. I currently place mostly Nobel implants followed by Astra implants. One article shows that after the one year mark the bone loss for an Astra implant was the same as a Branemark implant but the key is that this was after close to 1.75mm of bone loss with the Branemark vs the 0.4mm of bone loss with the Astra implant

After injury to the orbital bony vault a common sequelae is enophthalmos. This article reviews a way of assessing patients radiographically to predict when enophthalmos will occur and when to do surgery.
After any surgery including oral surgery postoperative shivering can occur. These patients may be hypothermic but they may not be as well. This article review does a nice job of going into the incidence, causes, risks and solutions for this common occurrence.
